ABOUT THE SERVICE  :    Registration of Live Births, Deaths and Marriages within the                                          30 Day reglementary Period

FRONTLINE SERVICE     : Late Registration of Birth, Death, Marriage or any Civil
                                        Registration Documents

ABOUT THE SERVICE 

A ten (10) day posting period must be observed before the Document applied for is released. Only application with complete Documents will be processed.

BASIC REQUIREMENTS :                      

1. Negative Certification from NSO on the concerned event  (Birth, Death or Marriage)
2. Affidavit of two  (2) disinterested person
3. Marriage Certificate if the registrant is married

Birth   : Any two (2) of the following:

1. Baptismal Certificate/Certificate of Dedication
2. Voter Registration Record
3. School Record
4. Any Valid Identification Card

Death: 

1. A transcription from the records of the hospital where the Event took place or a copy of the attending physician

Marriage:

1. A transcription from the church/court/office where the event
Was solemnized.
2.The client’s original copy if available.

Fees to be Paid  : Php 130.00

2. Parental Consent (18 – 20) or advice (21 – 24)
3. Pre-marriage counseling certificate
4. Divorce or annulment decree, if applicable
5. Death Certificate of Spouse, if widowed
6. Affidavit of Legal Capacity to Marry, for foreigner

ABOUT THE SERVICE 

Out-of-Town Registration / Reporting applies to persons whose Vital events (birth, death, marriage) took place outside the  Municipality but are not indicated in the Registry Books of that Place and in the records of NSO.

REQUIREMENTS :

2.Negative Certification from the Office of the Municipal   
   Civil Registrar where the event took place
3.Affidavit of two (2) disinterested persons
4.Postal Money Order (minimum of Php 300.00)

ABOUT THE SERVICE 

Legal Instruments are documents (Affidavit of Acknowledgement and Legitimation and Affidavit to Use the Surname of the Father under R.A. No. 9255) that are required to update the birth record of the registrant.

REQUIREMENTS

2.Affidavit of Acknowledgement (3 copies) – for
   Acknowledgement
3.Affidavit of Legitimation (3 copies) – for Legitimation
4.Affidavit to Use the Surname of the Father (3copies) if

                                          Correction of Clerical Error under RA No. 9048

ABOUT THE SERVICE  : This service intends to enable the Change of the First Name of the client when the registered name in the Birth Certificate is ridiculous, tainted with dishonor, extremely difficult to write or pronounce.

REQUIREMENTS   : Basic
